The following forms must be signed and returned prior to your child's first day of attendance:
Enrollment Form
Permissions and Agreements Form
Emergency Medical Authorization Form
Hospital/Dentist/Pediatrician Choice Form
You have 30 days from the first day your child attends to have your pediatrician fill out and sign these two last forms:
Immunization Form (must be signed by physician)
General Health Appraisal (must be signed by physician)
There is a non-refundable $100 registration fee and a non-refundable $100 tuition deposit to hold your child's spot. The $100 tuition deposit will be applied to your first month's tuition. Please return the form below with your registration fee and tuition deposit:

Payment can be made in person with cash or check. We are also able to accept payments using PayPal. Please be sure to enter your child's name and your contact information on the memo line of your check or on the PayPal site.
